Additional Services

Our re-entry assistance services help individuals successfully transition back into the community after incarceration, hospitalization, residential treatment, or other life-disrupting events. We provide support with connecting to mental health and substance use treatment, housing resources, employment readiness, life skills, care coordination, and community-based services to promote stability, recovery, and long-term success. 

 Toxicology screening is provided for all Substance Abuse Intensive Outpatient (SA IOP), Partial Hospitalization Program (PHP), and Mental Health Intensive Outpatient (MH IOP) services as part of ongoing treatment support. Screening may be used to help monitor substance use, support clinical decision-making, encourage accountability, and promote a safe and effective treatment environment. 

 Our care coordination and case management services help individuals access the support and resources needed to improve overall stability and well-being. We assist with linking clients to mental health and substance use treatment, medical care, housing, benefits, community resources, and other essential services while helping ensure continuity of care across providers and programs. 

 A comprehensive needs assessment is used to evaluate an individual’s mental health, substance use, medical, social, and functional needs to help determine the most appropriate services and supports. This process helps identify strengths, challenges, barriers, and treatment priorities so an effective, person-centered plan of care can be developed.

 A substance abuse assessment is used to evaluate an individual’s history of substance use, current symptoms, risk factors, and overall treatment needs. This process helps determine the appropriate level of care, identify related mental health or social concerns, and support the development of an effective, individualized treatment plan. 

 Crisis services provide immediate support for individuals experiencing urgent mental health or substance use-related challenges. These services are designed to help stabilize the individual, reduce risk, assess immediate needs, and connect them to the appropriate level of care and ongoing support.
